
To err is human, but how clinicians respond to those errors can make all the difference. This session will explore the professional actions healthcare providers can take following a medication error or clinical mistake to minimize harm, promote transparency, and prevent future occurrences. The discussion will also reflect on the powerful lessons from Japan Airlines Pilot Capt. Kohei Asoh, who, after crash-landing an aircraft in San Francisco Bay in 1968, candidly admitted his mistake by saying, “As you Americans say, I Effed up.” His honesty has since become a case study in accountability and leadership.
